What makes an HVAC space a permit-required confined space?

A space is permit-required when it has limited entry and exit, is not designed for continuous occupancy, and contains a serious hazard such as an oxygen-deficient or toxic atmosphere, an engulfment risk, or a configuration that could trap or asphyxiate an entrant. Many mechanical rooms, attics, and ducts qualify.

What is the minimum safe oxygen level for entry?

OSHA considers an atmosphere oxygen-deficient below 19.5% and oxygen-enriched above 23.5%. Refrigerant leaks, combustion, and rusting can pull oxygen below the safe level, so the atmosphere must be tested before and during entry.

Why are so many confined space deaths rescuers?

Because coworkers instinctively rush in to help a collapsed entrant without recognizing that the same atmosphere will overcome them. Roughly 60% of confined space fatalities are would-be rescuers, which is why a rescue plan and trained rescue services are required before entry.
